In accordance with House Bill 4224 (89th Regular Session), which went into effect on September 1, 2025, this page provides detailed instructions for consumers regarding health care records access, contacting the licensing authority, and filing complaints.
Requesting Your Health Care Records
You have the right to request and receive copies of your health care records. Here's how to request your records from Hope on the Horizon TX LLC:

- Include required information: Your request should include:
- Your full name and date of birth
- Dates of service (if requesting specific records)
- Your signature (for written requests)
- Preferred method of delivery (email, mail, or in-person pickup)
- Response time: We will respond to your request within 15 business days as required by law. If additional time is needed, we will notify you in writing.
- Fees: There may be reasonable fees associated with copying and providing records. We will inform you of any fees before processing your request.
