In accordance with House Bill 4224 (89th Regular Session), which went into effect on September 1, 2025, this page provides detailed instructions for consumers regarding health care records access, contacting the licensing authority, and filing complaints.

Requesting Your Health Care Records

You have the right to request and receive copies of your health care records. Here's how to request your records from Hope on the Horizon TX LLC:

How to Request Your Records

  1. Submit a written request: You can request your records in writing by:

    • In Person: You may also request records during your appointment

  3. Response time: We will respond to your request within 15 business days as required by law. If additional time is needed, we will notify you in writing.
